Patient Forms

Upon entering our office, our front desk associate will welcome you and have you sign in. If you did not get a chance to fill out the paperwork online, you are welcome to do that in our waiting area. However, it will save you time if you are able to submit the paperwork before your first visit. It is wise to bring any diagnostic images, reports, or other healthcare related files to your first visit if they are recent within the year.

Consultation

Next, you will have a consultation with one of our doctors to discuss your reason for coming to the office. It is designed for the doctor to decide whether your case is one worth exploring from a chiropractic standpoint and to determine if we are a good fit for you.

Examination

If we think we can help and and you decide to work with our office, our doctors will perform a complete chiropractic, neurological, orthopedic, postural, and physical work up to determine what is causing your problem.

X-Ray Studies

Depending on your condition and health history, we may recommend X-rays. These help us develop the most effective treatment plan for you, and alert us of any serious spinal conditions. Most people are amazed once they see their X-rays up close. In some cases, they can even identify their misalignments and degeneration themselves.

Report of Findings

Once the information is collected and examinations are complete, the doctors will follow up with you and report all findings and answer the following questions:

  • What is wrong?

  • Can you fix it and how?

  • How long will it take?

  • What will my treatment cost?

After reviewing your health history, goals, and examining your spine and X-rays, the doctors will discuss recommendations and notify you if your condition requires care with other providers. They will provide the best treatment and wellness program for your needs.

Wellness Program

Many patients come to our office strictly wanting wellness care. This is much like taking care of the bones in your mouth. When your spine had been treated successfully and you are feeling and functioning well, it is important to maintain that correction. Most of the work, just like your dental hygiene, can be done at home on your own time. However, regular maintenance must be done in order to make sure your spine is healthy and moving properly. Wellness programs are designed to maintain proper spinal movement and alignment, reduce the likelihood of osteoarthritis and disc degeneration, and keep the nervous system feeling and functioning properly.
